Complaints
If you have any complaints about the way we handle your data, please contact our Data Protection Officer via e-mail. Should you be unable to resolve the matter with this officer, you have the right to file a complaint with the Personal Data Authority.
Checking ‘Enrollment by Name’ database
We provide urgent care to patients who are enrolled as regular patients at a general practice in our region. We also provide care to patients registered in another region. If you report to the GP Urgent Care Center (UCC) with an acute care question, we will retrieve information about you from your own family practice. We only do this if you have given us permission to do so. With the information from your own GP we can treat you better. We also send your GP a message about the treatment at the UCC. Your primary GP can then continue the treatment.
We want to be sure that we are informing the right GP about your treatment. Therefore, we check this in the ‘Enrollment By Name’ database, which is managed by the Stichting Inschrijving Op Naam (ION Foundation). In this database, GP practices themselves record who is registered with them (www.inschrijvingopnaam.nl). The database does not contain any medical information about you, only that you are registered with your GP practice.
Do you have questions about the database or the use of your data? If so, please contact the ION Foundation using the contact form on the website: www.inschrijvingopnaam.nl/contactformulier.
